Enclosed are three sets of detail plans, a plan approval worksheet, a fee equal to $180.00, and appropriate project summary sheets.

 

I, Richard C. Westerfield, P.E., Ph.D., certify that the detail plans have been reviewed in accordance with the contract agreement between the City of Columbus and the Director of the Ohio E.P.A. which was entered on June 1, 2006.

 

I further certify that the proposed detail plans are in conformance with the requirements of Chapter 6109, of the Ohio Revised Code and Chapter 3745-91 of the Ohio Administrative Code and are in conformance with the general plan submitted as agreed to in the said contract.  I hereby request and recommend approval from the Director of the Ohio E.P.A. for the proposed detail plans subject to the usual conditions without further review by the agency.  Please copy the Design Engineer, Name & Address, with the approval letter.
